Free Dental Checkup Mechanics

The consumers then will SMS their postal code to 58558and will receive contact details of dentists from the respective areas, following which they can make an appointment and avail of the offer. To schedule an appointment with a Dentist for a Free Dental Check-up:
- SMS your complete Postal Code to 58558 and you will receive contact details of dentists in your area where you can avail this offer (Standard value added service SMS rates apply).
- Schedule an appointment with the dentist.
- Fill out the form given by the Dentist and hand it over to the Dentist, along with this brush packing (with the sticker on it), when you go for the checkup.
Terms & Conditions
- This offer is valid till 30th April, 2012 only.
- The offer entitles only one person to claim one Free Dental Check-up per brush packaging with the Oral-B Free Dental Checkup sticker.
- Offer is valid at select dentists in select cities in India. See list of cities attached herewith.
- Only a free Dental Check-up will be provided under this offer. Expenses on further treatment and consultations will have to be borne by the consumer redeeming the offer.

- The company will not be liable for any losses or damages, direct or indirect resulting from delay or loss of SMSs in course of transit.
- The Dentists suggested for contact through SMS shall have the full right to refuse the free checkup in the event the form given by the Dentist is incomplete or unsigned.
